COVID-19 vaccination clinic at our Lake Branch

Lake County General Health District is offering a COVID-19 vaccine clinic on June 24 at Mentor Public Library’s Lake Branch.

Want to receive the COVID-19 vaccine? We’ve teamed with Lake County General Health District to host a vaccination clinic from noon to 4:30 p.m. on Thursday, June 24, at our Lake Branch.

It’s open to everyone 12 and older. Both the Moderna and Pfizer vaccines will be available, but 12- to 17-year-olds can only receive the Pfizer vaccine. Also, a parent or guardian must be with the minor at the vaccination appointment, and they must print and bring this signed consent form (also available in Español) and proof of the minor’s birth date to the appointment.

The second dose of the vaccine will be administered from noon to 4:30 p.m. on Thursday, July 22, at the Lake Branch.

Walk-ins are welcome, but you can also register to attend the clinic using ArmorVax. Those without email addresses or who need assistance making an appointment can call the Lake County General Health District at (440) 350-2554 between 8 a.m. and 4:30 p.m. Monday through Friday.
